INTAKE PROCESS

Pre-Interview

Complete the application thoroughly being sure to include the following information as it pertains to the teen:

  • All necessary signatures & a picture of the teen
  • Detailed behavioral history
  • Counseling summaries or evaluations (if prior counsel)
  • Recent IEP (individual education plan) if one exists
  • Court documentation of guardianship (if you are the court-ordered legal guardian)

Send the completed application and additional information by mail to the Admissions Coordinator c/o the Fold. You can also fax the application to us (802-626-1176) AND send the original by mail.

After reviewing the application and confirming that the Fold would be a good placement for the teen, the admissions coordinator will give you a call to either set up an interview time or to request missing information.

Interview

After the interview date has been arranged, you will be sent information about the actual interview time. The teen and his/her guardian are the only people allowed to attend the interview except on the rare occasion that someone else needs to attend as well (determined on a case by case basis). No siblings are allowed to attend interviews. The family will meet with the home staff, school staff, counselor, and Program Director to further determine if the Fold would be a good match for the teen. Before leaving the interview you will know if the teen has been accepted into the program.

Post-Interview / Intake

If the teen is accepted into the program s/he will be given a few days to decide if s/he wants to enter the program. The teen will be given a date to call the home and state his/her desire to come. If the teen says “yes”, you would then send to us the “Agreement to enter” form. The teen will then be placed on a waiting list until such time as there is an opening. During the waiting period, the home staff will be connecting with the teen weekly through phone calls, and will provide devotional material for the teen to work through while waiting to enter. Once an intake date has been determined, the Admissions Coordinator will send out an “intake packet” detailing all the required documents that must be received prior to the date of intake.
